Advanced Health Care Directives

You have the right to give instructions about your own health care.  You also have the right to name someone else to Living Willmake health care decisions for you. An advanced Health Care Directive form lets you do one or both of these things. It also lets you write down your wishes about donation of organs and the selection of your primary physician.

Completing an advanced health care directive takes the burden off your loved ones when they have to make a decision about what you would have wanted. It gives them the ability to make sure your wishes are carried out and followed. The form is fairly simple to complete, and there is no cost.
